Patient Advocate and Population Health Specialist

Key Responsibilities- Conduct thorough health needs assessments for members, identifying areas where they may require additional support or resources.
- Develop and implement personalized care plans to address the unique needs of each member, ensuring they receive the most effective care possible.
- Collaborate with internal teams to facilitate the delivery of high-quality care, including coordinating with healthcare providers, specialists, and other stakeholders as needed.
- Stay up-to-date on industry developments, best practices, and emerging trends in population health management to continuously improve the organization's approach.
- Engage in ongoing professional development to enhance skills and knowledge, staying current with the latest tools, technologies, and methodologies.
- Associates degree in a related field, such as education, communication, or healthcare, or equivalent work experience required.
- 2+ years of experience in customer service or a related field, with a proven track record of delivering exceptional results.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
- Ability to work independently with minimal supervision, as well as collaboratively as part of a team.
- Strong analytical and problem-solving skills, with the ability to think critically and make sound decisions.
- Bachelor's degree in a related field.
- Experience working in a population health or care management setting.
- Knowledge of medical terminology and healthcare systems.
$55,000 - $70,000 per year, depending on experience and qualifications.
