Responsibilities
  • Conduct thorough assessments of participants' physical and functional needs, as well as their psychosocial status and needs, home safety, and family caregiver burden.
  • Develop and implement comprehensive care plans that address the unique needs of each participant and their caregivers.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication.
  • Monitor participants' progress and adjust care plans as needed to ensure optimal outcomes.
  • Communicate with primary care providers and center nurses to coordinate medication, ancillary tests, and other services as required.
  • Direct the provision of personal care, nursing rehabilitation tasks, and other services provided by Home Health Aides and Geriatric Caregivers.
  • Document changes in participants' conditions and details of care provided, ensuring compliance with Agency standards and policies.
Qualifications

To be successful in this role, you will need:

  • A current NY State Registered Nurse License.
  • A NY Drivers License or NY State Issued ID.
  • An Associate Degree in Nursing, with a BSN preferred.
