Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Primary Care Nurse Specialist. As a member of our Primary Care Nursing Service division, you will provide high-quality patient care to Veterans and their eligible family members.

Key Responsibilities
  • Provide primary care to Veterans and their eligible family members using the Patient Aligned Care Team (PACT) model.
  • Assess, plan, implement, and evaluate care based on age- and cultural-specific components.
  • Coordinate care focused on Veteran transition through the continuum of care, Veteran and family education, Veteran self-management after discharge, and supporting factors that impact customer satisfaction.
  • Manage Secure Messages from Veterans and emails from consulting healthcare team members.
  • Conduct virtual or face-to-face review of Veterans at high-risk of hospitalization, initiate appropriate consultations per protocol, discuss care with specialty consultants, address needs for preventative or chronic disease care, communicate triage to PACT member, review non-VA records, address alerts, and complete follow-up calls with Veterans or family members.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • One year of clinical experience in Primary Care.
Working Conditions

This position requires moderate lifting, light carrying, pushing, walking, standing, kneeling, repeated bending, climbing, and ability to distinguish basic colors. The work environment is fast-paced and requires working closely with others.
