The Registered Nurse (RN) is assigned to the Primary Care Nursing Service division of Nursing and Patient Care Service and performs nursing duties for all Roseburg VA Health Care System sites of care. The RN provides primary care to Veterans and their eligible family members using the Patient Aligned Care Team (PACT) model. This care is provided in the ambulatory clinics located at the Roseburg main campus and Brookings VA Clinic. The services that are provided may include primary care, case/care management, and other specialty care as designated for the location also provide cross coverage for other areas as assigned by management. Primary Care is the provision of integrated, accessible healthcare services by clinicians who are accountable for addressing a large majority of personal healthcare needs, developing a sustained partnership with Veterans, and practicing in the context of family and community.

Duties may include, but not limited to:
Functions as a primary nurse for a defined group of Veterans for their shift.
Assesses, plans, implements, and evaluates care based on age- and cultural-specific components. Assumes responsibility for the coordination of care focused on Veteran transition through the continuum of care, Veteran, and family education, Veteran self-management after discharge, and supporting factors that impact customer satisfaction.
Intake assessment, health promotion, disease prevention, Women's Health, management of acute and chronic bio psychosocial conditions, referrals for specialty, rehabilitation, and other levels of care, follow-up and overall care management, and Veteran and caregiver education.
Provides Direct Veteran care including scheduled clinic visits, walk-in or urgent visits, group visits, telephone visits, incoming telephone requests, triage/place orders by protocol, and manage pertinent Clinical Recommendations.
Manages Secure Messages from Veterans and emails from consulting health care team members.
Conducts virtual or face to face review of Veterans at high-risk of hospitalization, initiate appropriate consultations per protocol, discuss care with specialty consultants, address needs for preventative or chronic disease care, communicate triage to PACT member, review non-VA records, address alerts and complete Follow-up calls with Veterans or family members.
Assesses, plans, implements, and evaluates care based on age- and cultural-specific components.
Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process that includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider.
