About the Role

We are seeking a skilled and compassionate Registered Nurse Home Health to join our team at Providence Benedictine Nursing Center. As a Home Health RN, you will provide high-quality patient care and support to individuals in their homes, promoting independence and improving health outcomes.

Responsibilities
  • Provide direct and indirect quality care for patients of all ages, following the nursing process and in accordance with the Nurse Practice Act.
  • Coordinate patient care activities with other members of the healthcare team in a collegial relationship.
  • Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure comprehensive care.
  • Conduct thorough assessments, identify areas for improvement, and develop strategies to address patient needs.
  • Provide education and support to patients and their families, promoting self-care and independence.
Requirements
  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • 1 year of Medical/surgical experience in an acute care setting.
  • 2 years of experience in acute care or community mental health setting for Mental Health RN work assignment.
Preferred Qualifications
  • Recent Home Health experience.
  • Hospice or Palliative Care experience for Palliative Care RN work assignment.
  • Point of Care experience.
