Job Summary
We are seeking a skilled Home Health RN to join our team at Providence Benedictine Nursing Center. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.
Key Responsibilities
- Provide direct and indirect quality care for patients of all ages, following the nursing process and in accordance with the Nurse Practice Act.
- Coordinate patient care activities with other members of the healthcare team in a collegial relationship.
- Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
- Conduct thorough assessments, identify patient needs, and develop effective care plans to address those needs.
- Collaborate with patients, families, and caregivers to ensure seamless care transitions and optimal patient outcomes.
Requirements
- Education to meet certification, license, or registration requirements.
- Oregon Registered Nurse License.
- National Provider BLS - American Heart Association.
- 1 year of Medical/Surgical experience in an acute care setting.
- 2 years of experience in acute care or community mental health setting for Mental Health RN work assignment.
Preferred Qualifications
- Recent Home Health experience.
- Hospice or Palliative Care experience for Palliative Care RN work assignment.
- Point of Care experience.
