We are seeking a skilled and compassionate Home Health RN Manager to join our team at Providence Home and Community Care. As a leader in our organization, you will be responsible for overseeing patient care services and personnel, ensuring the highest quality of care for our patients.

Key Responsibilities
  • Provide oversight of all patient care services and personnel, ensuring the delivery of high-quality care.
  • Make patient and personnel assignments, coordinate patient care, and ensure patient needs are continually assessed.
  • Develop, implement, and update individualized plans of care to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Recruit, train, and develop a team of skilled healthcare professionals to provide exceptional patient care.
Requirements
  • Bachelor's Degree in Nursing or related field.
  • Current Oregon Registered Nurse License.
  • National Provider BLS certification within 30 days of hire.
  • 3+ years of experience in home health care, with a minimum of 2 years in a leadership role.
