Providence is seeking a skilled Home Health RN to join our team. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes.

About the Role:

This is a full-time position with a competitive pay range of $59.32. The successful candidate will have a strong background in medical/surgical experience and a passion for delivering exceptional patient care.

Responsibilities:
  • Provide direct and indirect quality care for patients of all ages, according to individualized needs.
  • Coordinate patient care activities with other members of the healthcare team in a collegial relationship.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Monitor patient progress and adjust care plans as needed.
  • Collaborate with healthcare providers to ensure seamless care transitions.
Requirements:
  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License.
  • National Provider BLS - American Heart Association.
  • 1 year of Medical/Surgical experience in an acute care setting.
  • 2 years of experience in acute care or community mental health setting for Mental Health RN work assignment.
Preferred Qualifications:
  • Recent Home Health experience.
  • Hospice or Palliative Care experience for Palliative Care RN work assignment.
  • Point of Care experience.
