Job Title: Case Management Director

Job Summary:

The Case Management Director plays a pivotal role in the development and implementation of the case management program at Pipeline Health. This includes overseeing the case management team, developing and enforcing policies and procedures, and coordinating care with other healthcare providers and community resources. The Director ensures that the case management program is meeting its goals and objectives, and that it is compliant with all applicable regulations.

Key Responsibilities:

  • Lead the case management team in the development and implementation of case management policies and procedures.
  • Collaborate with other healthcare providers and community resources to ensure seamless care coordination.
  • Monitor and evaluate the effectiveness of the case management program, making adjustments as needed to ensure compliance with regulations.
  • Develop and maintain relationships with key stakeholders, including patients, families, and healthcare providers.
  • Stay up-to-date on industry trends and best practices in case management, applying this knowledge to improve program outcomes.

Requirements:

  • Minimum of five (5) years' experience in Clinical Nursing required.
  • Minimum of five (5) years' experience in Case Management, preferably in a hospital setting.
  • Minimum of three (3) years' experience in a Case Management Supervisory capacity or higher, required.
  • Critical thinking, service excellence, and good interpersonal communication skills.
  • A capacity to learn, synthesize, make critical judgments, work independently, place patients and families first, and collaborate with the team.

Licensure/Certifications:

  • Current RN license issued and/or recognized by the state Board of Nursing for the state of employment.
  • Basic Life Support (BLS) from American Heart Association (AHA), required.
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) credential, preferred.
