Job Summary

The Manager of Case Management plays a pivotal role in the development and implementation of the case management program at Memorial Hospital of Gardena. Reporting to the Director, this individual is responsible for overseeing the case management staff, creating and enforcing policies and procedures, and coordinating care with other healthcare providers and community resources.

This position requires a strong leader who can ensure the case management program meets its goals and objectives, while maintaining compliance with all applicable regulations. The ideal candidate will possess excellent critical thinking, service excellence, and interpersonal communication skills, with the ability to read and comprehend written instructions, demonstrate strong organizational skills, and utilize PC skills effectively.

Responsibilities
  • Oversee case management staff and ensure the program is meeting its goals and objectives
  • Develop and implement case management policies and procedures
  • Coordinate care with other healthcare providers and community resources
  • Ensure compliance with all applicable regulations
  • Collaborate with the team to achieve departmental operations and communications
Requirements
  • Minimum of five (5) years' experience in Clinical Nursing required
  • Minimum of five (5) years' experience in Case Management, preferably in a hospital setting
  • Minimum of three (3) years' experience in a Case Management Supervisory capacity or higher, required
  • Critical thinking, service excellence, and good interpersonal communication skills
  • Ability to read/comprehend written instructions, strong organizational skills, and PC skills
Licensure/Certifications
  • Current RN license issued and/or recognized by the state Board of Nursing for the state of employment
  • Basic Life Support (BLS) from American Heart Association (AHA), required
  • Certified Case Manager (CCM) or Accredited Case Manager (ACM) credential, preferred
