We are seeking a dedicated Registered Nurse Case Management Specialist for a travel opportunity within a Long-Term Acute Care (LTAC) organization.
Role Responsibilities:
The selected candidate will:
- Coordinate comprehensive care management to ensure effective resource utilization and adherence to external review standards.
- Deliver ongoing support and expertise through thorough assessments, care planning, implementation, and evaluation tailored to individual patient needs.
- Enhance patient management quality and satisfaction, promoting continuity of care and cost-effectiveness by integrating case management, utilization review, and discharge planning functions.
- Act as a patient advocate, focusing on resource utilization, discharge planning, and addressing the holistic needs of each patient.
Facility Requirements:
Applicants must meet the following criteria:
- Current BLS certification (must be included in the submission packet, American Heart Association certification required; RQI certifications are not acceptable).
- Previous experience in RN Case Management.
- Valid RN license in California.
- Availability for Monday to Friday shifts from 8 AM to 5 PM.
