PRIMARY FUNCTION:
This role is dedicated to the orchestration of services for children experiencing significant emotional challenges and their families.

PRIMARY DUTIES:

BILINGUAL POSITIONS MUST BE ABLE TO PERFORM ALL OF THE DUTIES LISTED BELOW IN ENGLISH AND SPANISH: 1.

Oversee all facets of service delivery for participants (transport participants, assemble child and family team, assess needs and strengths, develop an Individualized Recovery Plan (IRP), complete functional assessments annually, advocate, provide or secure services (including transportation), testify in Court as required, etc.).

E 2. Act as a liaison between the child/family and community service providers to ensure the acquisition of necessary services. E 3.

Supervise and negotiate the provision of services and agreements through the child and family team, modifying the IRP as necessary and the Service Authorization Request (SAR) when new services are introduced or existing services are terminated.

E 4.

Prepare, complete, and submit required documentation by established deadlines (Progress Notes, IRP, Crisis Plan, SAR, measurement instruments, Court mandated documents, etc.).

E 5. Foster and maintain effective communication, collaboration, and cooperation with all stakeholders (i.e., BMCW, probation officers, etc.). E 6. Adhere to legal, organizational, and contractual regulations and policies. E 7. Participate in all meetings and professional development activities and deliver presentations as directed. NE 8. Fulfill all responsibilities of a mandated reporter. E 9. Perform other related tasks as assigned. NE 10. Exhibit teamwork and collaboration.

PHYSICAL REQUIREMENTS:

Regularly required to drive, stand, sit, reach, stoop, bend, and walk. Infrequent lifting may be necessary.

WORKING CONDITIONS:
Work is generally conducted in both field and office environments. Frequent local travel; some state-wide travel may be required. Flexible hours are necessary based on program demands.

REPORTS TO:
Program Coordinator, as designated

MINIMUM QUALIFICATIONS REQUIRED:
1. Bachelor’s Degree from an accredited institution in Social Work or a related field (e.g., psychology, sociology, counseling). 2. Minimum of one (1) year of relevant experience providing mental health services. 3. Bilingual proficiency in Spanish and English is highly desirable (mandatory for bilingual roles). 4. Strong communication skills, cultural competency, and interpersonal relationship abilities. 5. Excellent critical thinking, highly developed organizational skills, and capacity to manage multiple priorities. 6.

Able to engage and communicate effectively with adults, families, and all team members to implement problem-solving strategies, understand issues, and provide crisis services in an ethical, respectful, and responsible manner.

7. Proficient in using Microsoft Office suite. 8. Willingness to work flexible hours, including evenings and weekends, depending on the needs of the youth/family and program requirements. 9. Valid transportation, Wisconsin Driver's license, state minimum auto insurance, and must meet La Causa, Inc. driving standards. 10. Must successfully pass all background checks.
