Job Summary:
The Lead Nurse Case Manager plays a critical role in overseeing the accuracy, completeness, and consistency of the case management team. This includes pre-admission, admission, concurrent, and retrospective reviews of all patients. The role involves coordination of all systems and services required for an organized interdisciplinary team approach to healthcare, ensuring quality, cost-effective care, efficient utilization of resources, and health services continuity across all patient age groups.
Key Responsibilities:
- Provide guidance on policies and procedures related to patient flow, discharge process, care coordination, human resource functions, and performance improvement strategies.
- Coordinate with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Develop and implement case management plans to address complex patient needs and ensure timely discharge.
- Collaborate with healthcare providers to ensure accurate and timely documentation of patient information.
- Participate in quality improvement initiatives to enhance patient care and reduce readmissions.
- Provide education and training to staff on case management best practices and hospital policies.
- Contribute to the development of policies and procedures related to case management and patient care.
Requirements:
- Licensure as a Registered Nurse in Maryland, or eligible for practice under Compact state agreements. BSN preferred.
- 5 years of clinical experience in a healthcare setting.
- Prior Case Management and Utilization experience required; CCM (Certified Case Manager) or ACM (Accredited Case Manager) preferred.
Preferred Qualifications:
- Master's degree in Nursing or related field.
- Experience with electronic medical records and hospital software systems.
- Strong analytical and problem-solving skills, with ability to prioritize tasks and work independently.
