Job Title: Lead Nurse Case Manager

Job Summary:

We are seeking a highly skilled and experienced Lead Nurse Case Manager to join our team at University of Maryland Charles Regional Medical Center. As a key member of our Case Management Services team, you will play a critical role in ensuring the delivery of high-quality, patient-centered care.

Responsibilities:

  • Assist the manager of Case Management Services in overseeing the accuracy, completeness, and consistency of the case management team.
  • Coordinate all systems and services required for an organized, interdisciplinary team approach to healthcare.
  • Assure quality, cost-effective care and efficient utilization of resources and health services across the healthcare continuum.
  • Provide guidance on policies and procedures related to patient flow, discharge process, and care coordination.
  • Provide input on human resource functions, performance improvement strategies, and staff orientation and mentoring.

Requirements:

  • Licensure as a Registered Nurse in the state of Maryland or eligible to practice due to Compact state agreements.
  • 5 years of clinical experience in a healthcare setting.
  • Previous Case Management and Utilization experience; CCM or ACM preferred.
  • Demonstrated knowledge and skills to provide care appropriate to the age of patients served.
  • Knowledge of managed care, discharge planning, utilization, and care coordination.
  • Ability to identify problems, develop a course of action, and follow through to resolution.
  • Excellent analytical and team-building skills, as well as the ability to prioritize and work independently.
