Case Monitor, Bureau of Mental Health/Assisted Outpatient Treatment
Job Summary:The City of New York is seeking a highly skilled Case Monitor to join the Bureau of Mental Health's Assisted Outpatient Treatment team. As a Case Monitor, you will play a critical role in ensuring the successful implementation of the Assisted Outpatient Treatment program, which provides comprehensive mental health services to individuals with serious mental illness.
Key Responsibilities:- Monitor consumer adherence to court orders and treatment plans
- Verify consumer services monthly and ensure timely completion of treatment plans
- Document changes to treatment plans and significant events in accordance with policy and procedure guidelines
- Collaborate with treatment providers and hospitals to ensure seamless care coordination
- Participate in case conferences to discuss consumer eligibility and progress under the court order
- Masters Degree in Social Work from an accredited school of social work
- Valid Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license issued by the New York State Department of Education within one year of appointment
