Job Overview

GENERAL PURPOSE

Facilitates outreach and engagement initiatives for non-enrolled adults experiencing homelessness, aligning with the objectives of the Homeless Intervention Program's designated demographic.

KEY RESPONSIBILITIES

The responsibilities outlined below serve as examples of the diverse tasks that may be undertaken. The absence of specific duties does not preclude their inclusion in this role if the work is similar, related, or logically associated with this position.

  • Commits 30 hours weekly to outreach efforts in various locations, including shelters, service organizations, and areas frequented by homeless individuals.
  • Re-establishes contact with clients who have been out of touch for 30 days or longer.
  • Provides ongoing outreach to clients who have voluntarily exited the program and are still at risk.
  • Maintains an active outreach caseload of 15 non-enrolled individuals.
  • Manages a caseload of 3-5 enrolled clients as necessary.
  • Conducts a minimum of 30 face-to-face interactions weekly with individuals targeted for outreach services.
  • In the absence of the Team Leader, offers guidance and support to clients as required.
  • Assists clients in identifying and working towards their personal goals.
  • Advocates for clients and their families in accessing mental health services, medical care, housing, food, and clothing as needed.
  • Creates individualized service plans tailored to each client.
  • Guides clients in developing essential life skills, including social interaction, financial management, personal care, and job preparedness.
  • Collaborates effectively within the team to deliver comprehensive case management services to a defined group of clients.
  • Provides immediate counseling that is beneficial to clients and aligns with the program's philosophy.
  • Ensures the safety, health, and overall well-being of clients.
  • Offers after-hours support to program clients as needed.
  • Provides assistance to colleagues as required.
  • Adheres to the policies and procedures of Turning Point Community Programs.
  • Participates in all team and agency meetings unless prior approval for absence is obtained from the Team Leader and Program Director.
  • May be required to drive for company-related activities and transport clients or other eligible individuals.

QUALIFICATIONS

MINIMUM:

An Associate's Degree in Social Work or a related field, along with a minimum of six (6) years of diverse experience as a consumer or provider of mental health services is preferred.

OPTIMUM:

A Bachelor's degree in Social Work or a related field and four years of experience working with homeless populations.

LICENSES; CERTIFICATES; SPECIAL REQUIREMENTS

  • Possession of a valid California driver's license and current vehicle insurance/registration if driving; and,
  • Access to reliable transportation that meets safety inspection standards if over five years old, excluding all forms of two-wheeled transport including bicycles, mopeds, and motorcycles.
