Outreach Specialist

Turning Point Community Programs is seeking a dedicated Outreach Worker to join our Community Outreach Recovery and Empowerment (Core-Madison) program in North Highlands, CA. As an Outreach Worker, you will play a vital role in connecting with homeless individuals, providing support, and helping them achieve their goals.

Key Responsibilities- Conduct street and community outreach to homeless shelters, service agencies, and other locations where homeless individuals spend time.
- Reconnect with clients who have lost contact with staff for 30 or more days.
- Provide follow-up and continued outreach to clients who have voluntarily discharged from the program and remain at risk.
- Maintain a continuous outreach caseload of 15 non-enrolled individuals and 3-5 enrolled members as needed.
- Make at least 30 contacts per week with clients.
- Provide leadership and direction for clients in the absence of the Team Leader.
- Assist clients in accessing and receiving mental health treatment, medical care, housing, food, and clothing as indicated.
- Develop individual service plans for each client.
- Coach clients in developing social skills, money management, personal hygiene, and job readiness.
- Interact productively with the team to provide holistic case management services to an identified group of clients.
- Provide on-the-spot counseling and attend to the safety, health, and well-being of clients.
- Provide after-hours coverage and support to other staff members as needed.
- Bachelor's degree in Social Work or a related field and four years of experience working with homeless individuals.
- Associates degree in Social Work or a related field and a minimum of six years of varied experience as a consumer of mental health services or as a provider of mental health services.
- Valid California driver's license, current vehicle insurance, and a driving record acceptable to Turning Point's vehicle insurance company.
- Possession of a safe and reliable form of vehicular transportation, excluding motorcycles.
This is a full-time position working Monday through Friday, 8:00 am to 4:30 pm. The successful candidate will be required to drive on company business and may transport clients or other individuals who meet the criteria for the target population.
