Job Summary

As a Registered Nurse with Duke Health, you will provide professional nursing care for patients in their homes or alternate settings. You will work collaboratively with the interdisciplinary team to assess, plan, implement, and evaluate patient care.

Key Responsibilities
  • Assess patient needs and develop individualized plans of care
  • Provide education to patients and families on disease management and self-care
  • Collaborate with the healthcare team to ensure seamless care transitions
  • Document patient care and outcomes in accordance with agency policies and regulations
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Current RN license in NC or compact RN licensure from participating state
  • Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only)
  • 1-2 years of med-surgical, critical care or home health/hospice/infusion experience
  • Valid driver's license and current automobile insurance coverage
Preferred Qualifications
  • Prior experience in Home Health, Hospice or Infusion
  • Prior OASIS and PDGM experience preferred for home health
