Job Summary:
As a RN Care Coordinator for Home Health and Hospice, you will provide patient care coordination for patients in their homes or hospice settings. You will educate physicians and staff, facilitate continuity of care, and participate in interdisciplinary teams to establish and implement home care or hospice needs.
Key Responsibilities:
- Communicate patient information and coordinate staff availability with the home health or hospice intake department.
- Coordinate with patients, families, physicians, and staff to determine the appropriateness of home care referrals.
- Educate referral sources and hospital staff on home health or hospice services and regulatory requirements.
- Gather demographic and medical information to develop a comprehensive home health or hospice care referral and plan.
Requirements:
- Graduation from a state-accredited school of professional nursing; BSN preferred.
- Enrollment in an accredited program within four years of employment and obtain a Bachelor's degree in Nursing or a Master's degree in Nursing within seven years of employment date.
- Licensed to practice as a Registered Nurse in the state of North Carolina.
- Two years of home health, public health, or other health-related experience.
