Job Title: RN Case Manager - Skilled Nursing

Care Advantage Inc. - Skilled Care Division is seeking a highly skilled and compassionate Registered Nurse Case Manager to join our skilled nursing team. As a key member of our interdisciplinary team, you will provide direct care and coordinate with healthcare professionals to ensure our patients receive the highest quality care in the comfort of their own homes.

Job Summary

We are looking for a dedicated and passionate RN Case Manager to oversee all aspects of skilled nursing care for our patients in the Goochland, Glen Allen, Maidens, Mineral, VA, and surrounding areas. Responsibilities
  • Provide direct care to patients in their homes, including assessments, care planning, and coordination with healthcare professionals.
  • Oversee all aspects of skilled nursing care, including Oasis Assessments, Transfers, Resumption of Care, Discharges, and Significant Changes in Condition.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Develop and implement individualized care plans that meet the unique needs of each patient.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
Requirements
  • Graduate of an approved school of professional nursing.
  • Current and valid Virginia RN License.
  • Passionate and motivated to provide care within a patient's home.
  • Ability to exercise initiative and independent judgment.
  • Comfortable working collaboratively in cross-functional teams.
  • Ability to communicate effectively, both verbally and in writing, with all levels of an organization.
  • Ability and willingness to travel within a given territory.
  • Willingness to work with multiple patients in varying home locations.
Preferred Qualifications
  • Two (2) years' home health nursing experience, preferred.
  • Previous experience with Home Care Home Base EMR system strongly preferred.
  • Wound Care experience or Certification strongly preferred.
