Key Responsibilities- Participate in Care Management Huddles: Collaborate with the care management team to discuss patient cases, care plans, and address any challenges or concerns.
- Prioritize Workload: Assess the urgency and importance of various tasks and work with the care management team to ensure that high-priority tasks are addressed promptly.
- Delegated Tasks: Assist in the delegation of tasks assigned by clinicians, utilization managers, social workers, or care managers, and work closely with the healthcare team to ensure that delegated tasks are properly completed.
- Patient Care Transitions: Conduct activities to ensure patients are engaged in their care and have the necessary resources and support, including scheduling appointments and coordinating transportation as needed.
- Healthcare Navigation: Provide guidance and support to patients in navigating the complexities of the healthcare system, including explaining health insurance coverage, finding appropriate healthcare providers and services, and coordinating care between different healthcare providers and facilities.
- Resource Procurement: Assist in the procurement of services and/or equipment relevant to assigned departments' protocols.
- Care Transition Documentation: Facilitate the documentation required for care transitions, such as transcribing forms like FL-2 (Level of Care Determination) and PASRR (Preadmission Screening and Resident Review), and assist with necessary paperwork in an accurate and timely manner.
- Effective Communication: Maintain effective communication within the care management team and escalate any concerns or issues according to established guidelines, acting as a liaison between patients, clinicians, and other healthcare professionals to facilitate effective communication and resolution of any care-related challenges.
- Education: High School diploma or GED
- Licensure/Certification: No licensure or certification required
- Professional Experience: Minimum of 2 years of healthcare experience required
- Knowledge/Skills/Abilities: Strong organizational skills, excellent communication and interpersonal skills, proficiency in using healthcare software systems and electronic medical records, knowledge of care coordination principles, healthcare regulations, and documentation requirements, and ability to work collaboratively in a team environment and adapt to changing priorities.
This is a full-time position with a standard work schedule of 40 hours per week. The salary range is $27.16 per hour (hiring range), and pay offers are determined by experience and internal equity. This position is employed by UNC Health and is exempt from overtime.
