Job Summary

We are seeking a skilled Intake Specialist LPN to join our team at Humboldt Park Health. As a key member of our Behavioral Medicine Services department, you will play a critical role in ensuring seamless patient admissions and referrals.

Key Responsibilities
  • Manage Inquiries and Referrals: Respond to all inquiries related to admission referrals from internal and external customers, documenting all interactions on designated forms.
  • Initial Screening and Verification: Conduct initial screenings for admission requests, collecting preliminary demographics and verifying insurance benefits.
  • Collaboration with ER Physician: Assist the HPH ER physician in determining eligibility for admission based on established admission criteria.
  • Outpatient Referrals: Provide assistance to the ER attending in directing outpatient referrals for ER patients when necessary.
  • Pre-Admission Coordination: Coordinate all pre-admission activities with the BMU nursing unit for patients being considered for admission, including validation of required legal paperwork and gathering of pertinent information.
  • Admission Entry and Documentation: Complete admission entry paperwork with patients and maintain various logs of activities.
  • Performance Improvement Initiatives: Participate in department/intake performance improvement initiatives.
  • Cross-Training Opportunity: Be cross-trained to serve as a medication nurse on the inpatient BH units.

  • Must pass physical, drug screening, and background check.
