Certified Practical Nurse - Home Health Specialist

At TotalMed, we are seeking a skilled and compassionate Certified Practical Nurse (LPN) to join our home health care team. As a key member of our team, you will provide high-quality patient care in the comfort of their own homes.

About this Role:
Provide personalized nursing care to patients in their homes
Develop and implement individualized care plans to meet patients' unique needs
Collaborate with other healthcare professionals to ensure comprehensive care
Conduct thorough assessments to identify patients' needs and develop effective care strategies

Required Qualifications:
Current licensure as a Licensed Practical Nurse (LPN) in Maine
Minimum 1 year of experience in home health care or a related field
Strong communication and interpersonal skills
Ability to work independently and as part of a team
