MaineHealth is a leading healthcare provider in Maine and New Hampshire, dedicated to delivering high-quality patient care. We are seeking an experienced Registered Nurse to join our Home Health team as a Care Coordinator RN.

**Job Summary:**

The Care Coordinator RN will provide highly reliable, evidence-based, patient-family centered quality care through assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of the effects of nursing care. The ideal candidate will have greater than one year of prior RN experience and medical/surgical or prior home care experience.

**Key Responsibilities:

- Provide comprehensive care to patients in their homes, including assessments, treatments, and education on disease management and self-care.

- Develop and implement individualized care plans that meet the unique needs of each patient.

- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless transitions of care.

- Educate patients and families on medication management, disease prevention, and wellness strategies.

**Requirements:**

- Current license to practice as a Registered Professional Nurse in the State of Maine.

- Bachelor's degree in Nursing (BSN) preferred.

- At least one year of experience as a Registered Nurse in a hospital or home care setting.

- Strong analytical and problem-solving skills, with ability to prioritize tasks and manage time effectively.

- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.

**Benefits:**

We offer a competitive salary of $73,000 - $100,000 per year, depending on experience, plus benefits, including paid parental leave, flexible work policy, student loan assistance, training and education, and well-being resources for you and your family.
