This is a part-time professional nursing role within the clinical environment at the Department of Health.

The individual in this role will deliver direct public health intervention services to clients, focusing on areas such as Family Planning, Sexually Transmitted Infections (STI), HIV/PrEP, Tuberculosis (TB), and Refugee Health programs. This will be executed in accordance with established protocols and collaborative practice agreements.

The position necessitates a solid understanding of the public health framework and the ability to engage effectively with patients, healthcare professionals, and diverse community members.

Key Responsibilities:

  • Conduct assessments, evaluations, and management of patients following current professional standards and medical guidelines.
  • Maintain accurate documentation of all observations and actions in line with program requirements.
  • Gather and analyze patient data, including medical histories and physical assessments.
  • Deliver preventive health services aligned with programmatic standards.
  • Prescribe medications and ensure proper documentation in electronic health records.
  • Facilitate referrals to physicians or specialists as necessary.
  • Provide educational resources to patients and their families regarding diagnoses and care plans.
  • Offer guidance on family planning methods and reproductive health.
  • Administer pregnancy tests and provide counseling.
  • Collaborate with specialists to ensure comprehensive treatment for STIs.
  • Adhere to established clinical guidelines and protocols.
  • Work with Refugee Health nurses to conduct physical exams and follow-up referrals.
  • Document all actions in a timely manner within the Health Management System.
  • Address patient inquiries and concerns effectively.

Clinical Care Coordination:

  • Engage in public education and outreach to promote Department of Health services.
  • Contribute to the planning and achievement of community health objectives.
  • Participate in meetings and conferences to support program initiatives.
  • Utilize quality management practices to enhance service delivery.
  • Assess community health needs and recommend service improvements.
  • Actively engage in performance enhancement initiatives.
  • Serve as a mentor and role model for colleagues and students.

Professional Development:

  • Act as a preceptor for nursing students.
  • Demonstrate knowledge in the prevention and management of public health concerns.
  • Utilize technology effectively for patient care and documentation.
  • Possess a degree in nursing and licensure as an Advanced Practice Registered Nurse.
  • Experience in Family Planning or STD/HIV programs is preferred.

Additional Information:

This role may require participation in emergency duties, including working in shelters or responding to public health emergencies.
