StarCare Specialty Health System is seeking a highly skilled Master's Level Social Worker to join its Aging and Disability Team at Silver Star. As a key member of our team, you will provide social work services to promote the mental and social health of participants through assessment, treatment, teaching, and counseling.
Responsibilities- Conduct comprehensive initial assessments and reassessments on a semi-annual basis to develop and implement individualized care plans.
- Provide social work services, including basic casework, consultation, and liaison services with community resources.
- Facilitate communication between participants, their families, and the care team to ensure seamless coordination of care.
- Develop and facilitate participant councils to promote dialogue and collaboration among participants, caregivers, and staff.
- Collaborate with the interdisciplinary team, physicians, and other related staff to ensure comprehensive care.
- Provide leadership and training to care team members to ensure continuity and coordination of care and staff development.
- Monitor and ensure regulatory compliance of program quality while maintaining an optimal level of independence for participants.
- Master's degree in social work from an accredited institution.
- Current Texas License as a Master Social Worker (LMSW) or a related field.
- At least one year of experience working with a frail or elderly population.
- Meet the standardized set of competencies for the specific position description before working independently.
- Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
