Job Summary

StarCare Specialty Health System is seeking a skilled Master's Level Social Worker to join its Aging and Disability Team. The ideal candidate will provide social work services to promote mental and social health of participants through assessment, treatment, teaching, and counseling. Key responsibilities include comprehensive initial assessments, reassessments, plan of care, and coordination of 24-hour care delivery. The successful candidate will also implement social work care plans, coordinate with community services and supports, and provide basic casework and consultation for participants.

Responsibilities
  • Provide social work services to promote mental and social health of participants.
  • Conduct comprehensive initial assessments and reassessments.
  • Develop and implement social work care plans.
  • Coordinate with community services and supports.
  • Provide basic casework and consultation for participants.
  • Facilitate communication between participants, families, and the care team.
  • Develop and facilitate participant councils to promote dialogue among participants, caregivers, and staff.
  • Provide assessments, family and individual support, and serve as a liaison with community resources.
  • Facilitate discharge planning and post-discharge arrangements.
  • Collaborate with the interdisciplinary team, physicians, and other related staff.
  • Provide leadership and training to care team members.
  • Monitor and ensure regulatory compliance of program quality.

Requirements
  • Master's degree in social work.
  • Current Texas License as a Master Social Worker (preferred).
  • At least one year of experience working with a frail or elderly population.
  • Meet standardized competencies for the position description.
  • Be medically cleared for communicable diseases and have up-to-date immunizations.
