Aveanna Healthcare LLC: Private Duty Nurse - LVN

Responsibilities:
  • Implement and manage a patient's prescribed plan of care, ensuring seamless transitions and continuity of care.
  • Conduct thorough patient assessments, coordinate care, and administer prescribed medications, treatments, and therapies as needed.
  • Document all care provided in our electronic system of record, ensuring accuracy and compliance with regulatory requirements.
  • Provide education and training to family members on health promotion, disease management, and medication administration.
  • Collaborate with the care team to address patient concerns, changes in condition, and other issues that may impact patient care.
Requirements:
  • Active, unencumbered license (RN/LPN/LVN) in the state where you will practice.
  • Current CPR certification from a recognized organization.
  • TB skin test (current within the last 12 months).
  • At least six months of prior hands-on nursing experience (not required but preferred).
  • Reliable transportation.
