IDD Community Team Lead

Key Responsibilities:
- Manage a team of TBS Specialists by providing professional support, training, and mentorship.
- Manage a caseload of clients at 30% productivity.
- Connect clients with community resources.
- Ensure treatment/behavior plans are followed.
- Complete and submit required documentation.
- Communicate and follow up on client status.
- Collaborate with therapy, counseling, and psychiatric services providers.
Requirements:
- Bachelor's degree in Social Work or a related field.
- Experience in community-based case management and working with the IDD population.
- Managerial experience preferred.
- Valid driver's license, car insurance, and reliable transportation.
- Willingness to travel throughout the assigned service area.
