LPN Community Liaison

About the Opportunity:
We are seeking a skilled LPN to join our regional treatment team as a Community Liaison. This individual will work closely with the behavioral health provider, primary care providers, and other community agencies to coordinate multiple health care services.
Key Responsibilities:
- Assist the RN Care Coordinator with identifying individuals at risk and coordinating referrals to public and private health facilities.
- Participate in an interdisciplinary treatment team and utilize resources from other community agencies to achieve treatment goals.
- Act as liaison between healthcare providers and service agencies to carry out referral instructions.
- Make home visits to administer medication and assess individual's condition of health.
- Accompany clients to medically complex medical appointments and provide supportive interaction and education.
Requirements:
- Graduation from an approved school of Practical Nursing leading to the attainment of Practical Nurse license.
- A minimum of one year's experience working with individuals who have a severe or persistent mental illness.
- Current knowledge of psychotropic medications, including side effects.
- A car and a valid Ohio driver's license and insurability through the Agency's insurance carrier.
