Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments, medical history reviews, and provide personalized care recommendations to patients in their homes.

Key Responsibilities
  • Conduct adult and geriatric assessments, medical history reviews, and provide diagnosis and treatment recommendations
  • Work collaboratively with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care
  • Provide health education and counseling to patients and their families
  • Collaborate with primary care physicians to ensure seamless care coordination
  • Conduct assessments in a variety of settings, including home visits, skilled facility visits, and telehealth visits
Requirements
  • Master's degree in nursing or a related field
  • Current RN and NP or PA licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
  • Strong computer skills and familiarity with EMR software
  • Excellent verbal and written communication skills
