Matrix Medical Network offers a range of clinical services and expertise that support primary care providers and at-risk health plan members. Our network of clinicians provides in-home assessments and care management services to improve health outcomes and identify chronic conditions.
Job SummaryWe are seeking a Nurse Practitioner or Physician Assistant to join our team for PRN/Part Time Home Risk Assessments. As a member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, as well as provide health education and collaborate with physicians and other healthcare professionals.
Responsibilities- Conduct in-home assessments and medical history reviews
- Diagnose and treat patients as needed
- Provide health education and counseling
- Collaborate with physicians and other healthcare professionals
- Document patient information and treatment plans
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
