Job Summary

Affinity Care of Ohio is seeking a skilled Registered Nurse to join our team as a Hospice Case Manager. In this role, you will be responsible for providing high-quality patient care and coordinating services to meet the needs of individuals and families in their homes and communities.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to determine hospice needs and develop individualized care plans.
  • Provide professional nursing care, including administering medications and treatments as prescribed by physicians.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in interdisciplinary team meetings to discuss patient care and make recommendations for improvement.
  • Supervise and delegate responsibilities to ancillary personnel as needed.
  • Stay up-to-date with industry developments and best practices through ongoing education and training.
Requirements
  • Graduate of a National League of Nursing-accredited school of nursing.
  • Current licensure in the state of Ohio.
  • Minimum of one year of experience as a professional nurse within the last three years, or a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license.
  • Preferred: minimum of two years' experience in public health, home care, or hospice nursing.
