Job Title: Hospice RN Case Manager

Job Summary:

i4 Search Group Healthcare is seeking a compassionate and dedicated Hospice Registered Nurse to provide exceptional end-of-life care to our terminally ill patients. As a Hospice RN Case Manager, you will be responsible for assessing patients' medical conditions, maintaining their comfort, and providing guidance and support to their loved ones.

Key Responsibilities:

  • Collaborate with healthcare teams to develop and implement patient care plans.
  • Monitor patients' vital signs and physical comfort levels, and adjust care plans as needed.
  • Administer pain management medications and perform prescribed treatments.
  • Provide emotional support and education to patients' loved ones and primary caregivers.
  • Maintain accurate and up-to-date medical records and charts.
  • Notify healthcare teams and family members of patients' deaths and provide bereavement support.

Requirements:

  • Active Registered Nurse (RN) license in the state of practice.
  • Bachelor of Science degree in nursing (BSN) preferred.
  • Accredited certification in hospice and palliative care required.
  • Two (2) years experience as a hospice nurse preferred.
  • Excellent analytical and diagnostic skills, as well as strong communication and collaboration abilities.
