Responsibilities- Develop and implement comprehensive social services plans to meet the unique needs of our residents.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and delivery.
- Conduct thorough assessments and develop individualized plans to address resident needs and goals.
- Provide emotional support and counseling to residents and their families.
- Stay up-to-date on relevant laws, regulations, and industry best practices to ensure compliance and excellence in care.
- Bachelor's Degree in Social Work from an accredited institution.
- Active LSW license in good standing.
- Minimum 2 years of supervised social work experience in a healthcare setting.
- Long-term care experience preferred.
