Position Details:

Job Title: Registered Nurse Hospice Case Manager

Location: Remote (Home Health)

Contract Duration: 8+ weeks

Compensation: Weekly Gross Pay Range $2080 to $2120 (Includes non-taxable travel/housing and M&I stipend for eligible candidates)

Mileage Reimbursement: $0.49/mile for travel between patient appointments

Unit: Home Health Hospice

Work Schedule: 40 hours/week, Monday to Friday, 8 AM – 5 PM

Position Requirements:

  • Minimum of 1 year as a Registered Nurse - Required
  • At least 1 year of recent Home Health Nursing experience within the last 3 years - Required
  • At least 1 year of recent Hospice Nursing experience within the last 3 years - Required
  • Experience with Home Care Home Base (HCHB) charting - Required
  • OASIS experience - Highly preferred
  • Valid Driver's License and Personal Auto Insurance - Required
  • Covid Vaccination or valid medical/religious exemption - Required

Education and Certification Requirements:

  • ASN or equivalent - Required
  • BSN or equivalent - Highly preferred
  • RN License for the state of application or Compact if applicable - Required
  • BLS Certification from AHA or American Red Cross - Required

Key Responsibilities:

  • Deliver and coordinate care for assigned hospice patients in the comfort of their homes.
  • Manage an assigned caseload effectively.
  • Develop and implement comprehensive nursing care plans, including Start of Care and admissions.
  • Support patient recovery and enable them to spend quality time with loved ones.
  • Educate patients and families on disease management to empower them in reaching healthcare goals.
  • Provide hospice nursing care within the standards and scope of nursing practice as defined by state regulations and corporate policies.
  • Ensure clarity of physicians' orders and proper documentation.
  • Continuously assess patient status and document relevant information.
  • Notify physicians of significant changes in patient condition.
  • Complete charting for all patient visits within 24 hours.
  • Offer bereavement support to families as needed.
  • Territory coverage includes surrounding areas.
