Job Title: Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at Lancaster General Health. As a Clinical Pharmacist, you will play a critical role in providing medication-related care to patients, collaborating with physicians and other healthcare professionals to evaluate drug therapy and improve patient outcomes.

Key Responsibilities:
  • Evaluate drug therapy based on acuity and potential for problems
  • Provide medication-related care to achieve definite outcomes
  • Collaborate with physicians and other healthcare professionals to improve patient outcomes
  • Conduct drug utilization reviews to assure quality of drug use
  • Develop criteria and make recommendations to the Pharmacy and Therapeutics Committee
  • Interpret and evaluate medication orders to ensure safe and effective medication use
  • Prepare and dispense medications in accordance with professional practice guidelines and regulatory standards
Requirements:
  • Pharm.D. or B.S. Pharmacy with equivalent relevant experience
  • Current licensure in the State of Pennsylvania
  • ASHP-Accredited Pharmacy Practice Residency or equivalent experience
  • Healthcare provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs
Preferred Qualifications:
  • Equivalent of three (3) years experience as a hospital pharmacist
