Key Responsibilities:
  • Evaluate drug therapy and provide recommendations to healthcare professionals to improve patient outcomes and reduce costs.
  • Conduct drug utilization reviews to ensure the quality of drug use and identify opportunities for improvement.
  • Provide education and training to healthcare professionals and patients on the safe and effective use of medications.
  • Develop and implement medication therapy plans to achieve specific patient outcomes.
  • Collaborate with healthcare professionals to identify and address medication-related issues and concerns.
Requirements:
  • Pharm.D. or B.S. Pharmacy degree with equivalent relevant experience.
  • Current licensure in the State of Pennsylvania.
  • ASHP-Accredited Pharmacy Practice Residency or equivalent experience preferred.
  • A health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs.
Preferred Qualifications:
  • Equivalent of three (3) years experience as a hospital pharmacist.
