Key Responsibilities
  • Evaluate drug therapy and provide medication-related care to patients
  • Collaborate with physicians and other healthcare professionals to develop and implement medication therapy plans
  • Conduct drug utilization reviews to ensure safe and effective medication use
  • Provide education and training to healthcare professionals and patients on medication use and safety
  • Develop and implement policies and procedures for medication use and safety
  • Participate in quality improvement initiatives to improve patient outcomes and reduce costs
Requirements
  • Pharm.D. or B.S. Pharmacy degree
  • Current licensure in the State of Pennsylvania
  • ASHP-Accredited Pharmacy Practice Residency or equivalent experience
  • Healthcare provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs
Preferred Qualifications
  • Equivalent of three (3) years experience as a hospital pharmacist
