Pediatric Social Worker Job Description

At Dartmouth Health, we are seeking a highly skilled and compassionate Pediatric Social Worker to join our team. As a key member of our healthcare team, you will provide essential social services to patients and their families, ensuring they receive the support and resources they need to navigate complex healthcare situations.

Key Responsibilities:
  • Conduct thorough psychosocial assessments of patients and families to identify their unique needs and develop personalized plans to address them.
  • Provide counseling and support to individuals and families to help them cope with social and emotional challenges.
  • Coordinate community resources and services to meet the identified needs of patients and families.
  • Assist families in adapting to difficult and complicated dispositions involving complex care needs, multiple agencies, and linkage to resources outside the community.
  • Provide consultation, assessment, referral, and coordination in cases of suspected child or dependent adult abuse or neglect, or other forms of domestic violence.
  • Organize and lead support groups to promote social connections and emotional well-being.
  • Document objective, concise, and pertinent clinical data in the medical record.
  • Perform other duties as assigned by management.
Qualifications:
  • Master's Degree in Social Work (MSW) required.
  • Social work experience in an acute, home care, or ambulatory setting preferred.
  • Strong organizational skills, verbal and written communication skills.
  • Sound decision-making and judgment.
  • Time management skills.
  • Negotiation skills.
  • Proven ability to deal with conflict resolution.
  • Comfortable with basic computer skills.
  • Child protection experience preferred.
