Job Summary:

The Mental Health Therapist is a vital member of our interdisciplinary team, providing comprehensive and culturally sensitive social work interventions to patients. This role involves conducting psychosocial assessments, developing individualized treatment plans, and facilitating group and individual therapy sessions. The successful candidate will have a Master's Degree in Social Work (MSW) or Mental Health Counseling (MHC) and a minimum of two years of post-graduate experience in a medical or mental health setting.

Key Responsibilities:

Conduct thorough psychosocial assessments and develop individualized treatment plans for patients
Facilitate group and individual therapy sessions, including crisis interventions and stabilization
Collaborate with the interdisciplinary team to ensure excellent patient care and timely discharge planning
Maintain accurate and timely documentation of social work assessments, interventions, and communications with internal and external stakeholders

Requirements:

Master's Degree in Social Work (MSW) or Mental Health Counseling (MHC) from an accredited Graduate Program
Two years of post-graduate experience in a medical or mental health setting as a member of a multi-disciplinary team
NYS Department of Education License or Limited Permit License to practice as an LMSW, LCSW, LMHC
