Key Responsibilities
  • Conduct thorough needs assessments to identify clients' strengths and opportunities for growth
  • Develop and implement personalized care plans to address clients' medical and psychosocial needs
  • Provide education and support to clients to help them understand their diagnoses and treatment options
  • Collaborate with healthcare providers and community agencies to ensure seamless care coordination
  • Monitor clients' progress and make adjustments to care plans as needed
Requirements
  • Bachelor's or master's degree in Social Science, Social Work, or Human Services
  • Minimum of 1 year of case management experience
  • Knowledge of Ryan White Services and HIV/AIDS care
  • Bilingual in Spanish and English with strong verbal and written communication skills
