Qualifications:

  • Bachelor's or master's degree in psychology, counseling, or social work from an accredited institution, as required by state regulations.
  • A minimum of three (3) years of experience in substance use, mental health, or a related field.
  • Licensure or certification in one of the following:
    • LPCC
    • LSW
    • LMFT
    • LPC
    • LICDC
    • LIDC-CS
    • LCDC-II
    • LCDC-III
    • SWT (Registered Social Worker Trainee)
    • MFTT
    • CDCA

Key Responsibilities:

  • Collaborate with the clinical team in decision-making processes regarding patient care, including attending all clinical meetings.
  • Evaluate the counseling needs of patients.
  • Conduct assessments for substance abuse diagnoses using DSM criteria to determine the appropriate level of care based on ASAM criteria.
  • Gather relevant previous records pertinent to the current treatment episode.
  • Prepare the substance abuse counseling segment of the patient treatment plan.
  • Deliver substance abuse counseling services as outlined in the patient treatment plan.
  • Develop discharge and continuum of care plans to facilitate transitions to appropriate levels of care.
  • Actively participate as a member of a multidisciplinary team for assigned patients.
  • Document all counseling and educational services, assessments, reassessments, referrals, and follow-ups in the patient's clinical record.
  • Maintain a patient caseload, including all case management and utilization review updates, treatment planning duties, and clinical charting as required.
  • Coordinate support services for patients and their families, documenting services provided in the clinical record.
  • Reassess patients and update the substance use counseling portion of the treatment plan as necessary.
  • Facilitate family therapy sessions as assigned.
  • Engage with referral sources (e.g., legal, educational, employment) as needed.
  • Implement interventions for crisis-oriented issues using a multidisciplinary team approach for assigned patients.
  • Perform administrative and clinical functions as requested.
  • Attend team meetings and complete all required training courses in a timely manner.
  • Other responsibilities as assigned.
