Job Description: We are seeking a skilled Nurse Care Coordinator to join our team in Dallas, TX. The successful candidate will work closely with patients, families, and healthcare teams to coordinate care and ensure seamless transitions throughout the cancer care journey.
Responsibilities:
- Assess patient needs and develop individualized plans of care
- Coordinate care with other healthcare providers, including physicians, pharmacists, and social workers
- Develop and implement care plans that meet patient goals and preferences
- Maintain accurate and complete records of patient care
- Communicate effectively with patients, families, and healthcare teams
Requirements:
- Bachelor's degree in nursing or related field
- RN license and current registration with the State Board of Nursing in practicing state
- Current Basic Life Support (BLS) certification
- Minimum 1 year of experience in oncology nursing
