Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Metropolitan Family Services. As a key member of our North Team, you will play a vital role in providing exceptional support to our clients and helping them achieve their goals.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of clients to identify their needs and develop personalized service plans to help them overcome challenges and achieve optimal functioning in their communities.
  • Case Management: Provide supportive help, advocacy, and case management to individuals and families in coping with environmental and interpersonal relationship issues.
  • Community Resource Development: Develop a knowledge of community resources and help clients access them to meet their needs.
  • Collaboration and Communication: Work collaboratively with other program staff, community resources, and clients to ensure effective service delivery and achieve desired outcomes.
  • Reporting and Documentation: Maintain accurate records and reports to meet contract, grant, and program requirements.
Requirements
  • Education: Bachelor's Degree in Social Work or a related field from an accredited college or university.
  • Experience: Strong interpersonal and time management skills, with some direct service experience preferred.
  • Skills: Ability to work effectively with diverse individuals and groups, maintain confidentiality, and handle emergency and crisis situations.
