Position Overview

The Youth Housing Support Specialist (YHSS) will deliver comprehensive case management services to individuals facing homelessness within the Youth Homelessness Demonstration Project (YHDP). This role is primarily field-based, focusing on client-centered support that involves significant direct interaction with participants. The YHSS adheres to established evidence-based practices to foster safe and stable living conditions for clients. The specialist's expertise in relevant resources is designed to be housing-oriented, trauma-informed, and collaborative, working alongside families to help them achieve their aspirations.

This is a full-time, non-exempt position with a starting annual salary of $40,000.00, with adjustments based on relevant education and experience.

Key Responsibilities:

  • Manage a caseload of up to twenty-two youths referred through the Coordinated Entry System, typically averaging around 15.
  • Collaborate with participants to tackle barriers that may hinder their access to housing.
  • Implement core principles of Housing First, Trauma-Informed Care, and Motivational Interviewing.
  • Conduct necessary assessments with youth as part of the YHDP program intake process.
  • Provide strengths-based, adaptable, and responsive case management support to participants.
  • Engage with youth to define their personal goals and visions for themselves and their families.
  • Develop housing stability plans tailored to the identified goals for both adults and children, focusing on securing or maintaining housing, income, and other essential services.
  • Perform bi-monthly home visits and maintain weekly communication with young adults to monitor progress, coordinate services, and assist in achieving established goals or preventing crises.
  • Facilitate the housing placement process for each participant, acting as a liaison between youth, landlords, utility providers, and other relevant agencies.
  • Assist with move-ins to ensure participants have the necessary household items and a clear understanding of their responsibilities as tenants.
  • Connect participants with services and resources that support their safety, well-being, and long-term stability.
  • Network with community resources related to housing, employment, substance abuse treatment, mental health services, and training, making referrals as needed.
  • Obtain signed release of information forms and maintain regular communication with staff at treatment and vocational programs.
  • Complete data entry in the Homeless Management Information System (HMIS), including case notes within five business days of contact, self-sufficiency assessments, and participant updates.
  • Maintain accurate and detailed case files for each participant, including all intake documents, case plans, monthly budgets, and other program forms.
  • Participate in service coordination meetings, case conferences, and training programs.
  • Exhibit effective communication skills to build relationships with peers and participants.
  • Engage in monthly case conferences and other relevant activities within the Continuum of Care and/or Coordinated Entry System.
  • Travel and transport clients as necessary.
  • Adhere to the ethics, policies, procedures, and professional standards of IMPOWER, Inc.
  • Complete required essential learning trainings within designated time frames.
  • Fulfill additional tasks and responsibilities as assigned to support IMPOWER's vision and mission.

Qualifications and Experience:

  • Associate's Degree or equivalent college credits in a social services field; Bachelor's Degree preferred.
  • Two years of documented case management experience, with a preference for experience working with youth in a supportive role.
  • Successful background screenings through the Florida Department of Law Enforcement, State, and County.
  • Valid driver's license and auto insurance, along with a satisfactory motor vehicle driving record.
  • Negative result from a multi-panel drug screening.
