Job Description: As a Peer Navigator, you will support members experiencing homelessness with managing their health in the Long Beach area. Your responsibilities include building rapport, coaching, linking members to local services, conducting outreach, providing accompaniment, collaborating with staff, introducing self-management tools, and documenting all required information.
Required Skills and Qualifications: To succeed in this role, you should have a G.E.D./High School diploma or equivalent, 3+ years of work experience, direct work experience with patients experiencing homelessness, and excellent oral communication skills. You must also be able to prioritize multiple tasks, maintain reliable transportation, and travel 50% or more.
