Job Summary

The Commonwealth of Virginia is seeking a highly skilled Clinical Pharmacist to join our team in the Ambulatory Care and Cellular Therapy department. As a Clinical Pharmacist, you will be responsible for providing evidence-based pharmaceutical care to patients in the ambulatory clinical setting, with a focus on stem cell transplants and cellular therapy.

Key Responsibilities
  • Design, implement, and assess therapeutic plans for patients undergoing stem cell transplants and cellular therapy
  • Provide patient and caregiver education on medication management, laboratory monitoring, and supportive care optimization
  • Collaborate with multidisciplinary teams to develop and implement treatment plans
  • Participate in quality improvement initiatives and academic research
Requirements
  • PGY-1 residency required or equivalent clinical experience (3 years)
  • PGY-2 residency preferred for specialty areas
  • Commonwealth of Virginia State Board of Pharmacy License required
  • American Heart Association (AHA) Health Care Provider BLS certification required
