Assistant Director of Case Management

Job Summary:

The Assistant Director of Case Management plays a critical role in the management of the case management staff, overseeing hiring, termination, supervision, scheduling, and orientation of new staff. This position is responsible for submitting F1s and overseeing position control, as well as assisting in managing day-to-day operations.

The Assistant Director will attend daily operations huddles, long length of stay meetings at both campuses, escalation meetings, and post-acute partnership meetings as needed. This role will also co-facilitate case management staff meetings and provide administrative coverage for the Director of Case Management.

Key Responsibilities:
  • Assist with coordination of strategic and operational case management initiatives
  • Support customer and staff satisfaction, quality management, certification, and accreditation activities
  • Develop professional practice activities and oversee the SA system as it relates to case management
Requirements:

Bachelor's degree in Nursing, NYS Licensed/Registered (or eligible) RN, and a minimum of 5 years post-licensure acute care RN experience required. Minimum of one year Nursing leadership experience required. Prior Case Management experience and demonstrated competency in teaching/learning process, organizational skills, and computer literacy required. Excellent written/oral communication, leadership, organizational, interpersonal, and analytic skills required. CPR-BLS certification required by time of appointment.

Preferred Qualifications:

National Certification in Case Management, Care Coordination, or Transitional Care strongly preferred. Master's degree in Nursing preferred.

Work Schedule:

Monday-Friday: Days with some weekend responsibilities
