Job Summary:

The Assistant Director of Case Management will assist the Director of Case Management with responsibility and accountability for managing the case management staff, including hiring, terminating, supervision, scheduling, orientation of new staff, submitting F1s, and oversight of position control. This role will also assist in managing the day-to-day operations, including attending daily operations huddles, long length of stay meetings, and post-acute partnership meetings as needed.

This position will co-facilitate case management staff meetings and provide administrative coverage for the Director of Case Management. Specific duties will include assisting with coordination of strategic and operational case management initiatives, customer and staff satisfaction, quality management, certification and accreditation activities, and development of professional practice activities.

Oversight of the SA system as it relates to case management is also a key responsibility of this role.

Minimum Qualifications:

Bachelor's degree in Nursing, NYS Licensed/Registered (or eligible) RN, and a minimum of 5 years post-licensure acute care RN experience are required. A minimum of one year of nursing leadership experience and prior case management experience are also required.

Demonstrated competency in teaching/learning process, organizational skills, and computer literacy are essential. Excellent written/oral communication, leadership, organizational, interpersonal, and analytic skills are also required. CPR-BLS certification is required by the time of appointment.

Preferred Qualifications:
National Certification in Case Management, Care Coordination, or Transitional Care is strongly preferred. A Master's degree in Nursing is also preferred.
